Once you learn that you are pregnant, you will need to have your maternity leave paperwork submitted to the Business Office 3 months before your expected due date.
The documentation needed includes the following:
The
Leave of Absence Form and
PPL Form are available on the Dickson County Schools website, under Faculty & Staff - Frequently Used Forms, or under the Business Department - Important Links section.
To be eligible for PPL, support staff must have been employed in the Dickson County School System full-time for 12 consecutive months. Certified staff must have been employed as a teacher for the State of Tennessee in a public school system for 12 consecutive months. PPL consists of 30 paid days.
If you plan to take your PPL intermittently, you must submit a proposed schedule to the Director of Schools for approval.
While on maternity leave, if you are currently on a Dickson County Schools-sponsored insurance plan, your benefits will continue as long as you are on paid leave or PPL.
If you are on a Dickson County Schools-sponsored insurance plan and wish to add your newborn to your policy, you will have thirty (30) days from the date the baby is born to submit your enrollment request to the Business Office. You will need to submit a copy of the baby’s birth certificate (Mother’s copy is acceptable), along with the
insurance enrollment form, which can be found on the Dickson County Schools website, under Faculty & Staff - Frequently Used Forms, or under the Business Department - Important Links section on the right. The aforementioned form and documentation should be forwarded to the benefits department at:
[email protected] within the specified time frame.
Prior to returning to work from maternity leave, you must provide a physician’s note to the Human Resources Department for clearance. For a regular birth, the leave consists of 6 weeks. For a C-section birth, leave consists of 8 weeks.
If you want to begin your leave before the birth of your child, or want to come back to work earlier than previously planned, you must submit a letter to the Director of Schools for approval. A physician’s statement must accompany these requests. If you desire to extend your maternity leave, you must submit a letter to the Director of Schools for approval.
